Training bulletin: Collapsing Scissor lift Guardrails

Incident:

  • Worker was tasked with folding down a handrail system on a scissor lift to allow the machine to pass under a scaffold.
  • The lift handrail was “stuck” causing the worker to hold onto the collapsed midrail with their left hand, using force with their right hand.
  • The handrail struck the employees le
    ft third finger resulting in a fracture.

Cause:

  • Collapsing lift rails is a deviation of normal operations that is not covered in operator training.
  • Improper hand placement.
  • No impact resistant gloves.

Countermeasures:

  • Operators must consult equipment manual prior to any deviation of normal operations (i.e. Collapsing handrail system to allow clearance).
  • Stipulate that any task deviations be recorded on the PTP or JHA for your specific job site, especially if pinch point or line of fire issues are present for the deviation or non-normal procedure.
